At Addicus’ Legacy Dog Rescue (ALDR), our primary purpose is to find the right match for the animals so that both the humans and dogs are happy. Most of the animals that we help come from high-kill shelters. We have no way of knowing if they are sick or generally unhealthy until we get them to your home. We do everything we can to get as much behavior and health info as we can prior to pulling the dog.​

Please read the following and agree to the following statements by signing below​


I understand the foster animal(s) is the property of ALDR and I will not sell, trade or dispose of the animal, and must be returned to ALDR within 24 hours of request. I further agree that ALDR’s rights and to my foster pet are superior to mine.I agree not to transfer this dog(s) to the custody of another person, shelter, humane society, SPCA, or other entity without prior consent and permission of ALDR whether it be temporary or permanent.

I agree to provide the Authorized Representative from ALDR to access to my home and property to check on my foster pet, at any time while I am in possession of my foster pet.

I understand that I will be responsible for boarding expenses if needed. With advanced notice, we will try to find someone to pet sit your foster, however, we cannot guarantee that this will be available.

I understand that the dog is to be kept as an inside dog and will not live outside. They will sleep INSIDE my home in an appropriate area.

I understand that it is my responsibility to work with my foster on good home manners, potty-training, crate training, etc. if necessary. The goal is to get your dog prepared for a successful adoption.

I understand that I am expected to foster this dog(s) until a suitable applicant adopts the dog. If there are issues that arise with your dog and you would like ALDR to find another foster for your dog, we will work diligently to place the dog elsewhere as soon as possible, however, it may not be something ALDR can do immediately without notice.

Marketing and Adoption

I agree to stay connected with our volunteers on an on-going basis to share updated photos, bio information, personality traits, issues and concerns regarding your dog for marketing purposes. Sharing good photos and specific details about your foster will be the fastest way to find them their forever family.

I understand that anyone interested in adopting my foster dog or puppy (including a family member, friend, neighbor, or myself) will be required to go through the standard adoption process and that approval of candidates and placement of animals is determined by ALDR.

I understand that I have two (2) weeks to decide to adopt my foster dog ahead of other applicants. After that time, if we have an approved adopter, we will move forward with that adopter as first in line.

I agree to attend adoption events with my foster dog(s) and to schedule meet and greets with potential adopters as needed. I understand that ALDR volunteers will approve applicants and check their references before contacting me to schedule a meet and greet.

Although the foster family’s input is extremely valuable in the adoption process, I understand that the approval and ultimate placement of the dog is at the sole discretion of ALDR in the best interest of the dog.

Pet Health and Disposition

I understand that ALDR cannot guarantee the health or disposition of any foster animal. We do not have past records for these animals and there are some risks associated with taking in foster animals.

I understand that any animals that I currently have in my home and/or exposed to the foster dog(s) must be up to date on vaccinations in order to protect them from any disease that the rescue animal may have.

I understand that it is recommended that foster pets be kept isolated from other pets for a minimum of seven (7) days for their protection.

I understand that if I have small children, that I must teach them proper behavior around animals. Since ALDR frequently does not know anything about the animals that they rescue, I understand that extra caution must be used around small children and other animals. 

I understand that it is my responsibility to provide warm shelter, daily exercise, proper food (at my expense), and an adequate fence so that the dog cannot get out, and general responsibility for the dog(s) and their health.

I agree to take the dog(s) to all necessary vet appointments. If I would like to be reimbursed for these expenses, I understand that I will need to use one of ALDR approved medical providers.

I understand that ALDR will provide preventative flea/tick and heartworm medication at no charge as long as the medication is received directly from our organization. I understand that ADLR cannot reimburse me if I purchase on my own.

I understand that if the animal I foster is on medication, I will continue the medication as directed. If during the time I am fostering the animal requires medical attention, I will contact the ALDR first before getting any medical attention.

I understand that ALDR uses specific medical providers and I understand that they are the only approved providers of care for my foster. If I choose to take the animal to another vet, I will be responsible for all fees charged.

I will not declaw, crop ears, or crop tail of fostered pet.

I will adhere to all state and local animal laws and all foster animals will wear a collar with identification.

I will promptly notify ALDR of any signs of illness, behavioral issues or concerns, an inability to continue to foster, if the pet becomes lost, and/or if the pets bites someone.

Personal or Property Damage and/or Injury 

I agree that accidental animal bites or other injuries to humans and other animals do occur, and agree to hold harmless and indemnify, and protect Addicus’ Legacy Dog Rescue from any claim or suit filed by someone as a result of such incident. I understand that foster dogs must be on a leash at all times in public.

I understand that ADLR will not be held responsible if animal should damage or destroy property belonging to Foster Caregiver, Foster Caregiver household, or others, or if the animal shall transfer any disease, internal or external parasites to other animals and people in Foster Caregivers household.

I understand that if I am approved for fostering, this declaration represents a legal contract between me as the foster home caregiver and Addicus’ Legacy Dog Rescue.

I understand that if I am approved to foster an animal, I must abide by this agreement and that this agreement applies to any and all animals that I foster with Addicus’ Legacy Dog Rescue.

By signing below, I acknowledge that the information provided on this application is correct to the best of my knowledge. If at any time the information I have provided changes, I will provide the updated information to the Addicus’ Legacy Dog Rescue Foster Care Program. Please sign below to indicate your agreement and to be bound by these terms.


NOTE: You must contact your Vet office and let them know that we will be contacting them as a reference. We cannot approve you as a foster until we insure your current pets are up to date on vaccinations and treatment.
