Transport: Drop-off Instructions


  • Dog must be up-to-date on vaccines including:

    • Two rounds of DHPP for all dogs and puppies

    • Rabies for all dogs > 16 weeks old

    • Bordetella

    • A negative heartworm test for all dogs > 6 months old, unless they were recently treated for it (this information should be including on their vet records)

    • Spay/Neuter documentation for all dogs > 6 months old

  • A Health Certificate from a veterinarian is required for ALL dogs and puppies going on transport!

    • Must be issued 5 to 8 days before transport

    • The dog/puppy must be seen by a Vet in person to receive a Health Certificate 

    • Must fulfill all medical requirements outlined above.

If for any reason you are unable to drop your dog off for transport, please let your Adoption Coordinator know in advance, so we can arrange a volunteer to assist. You can also post on our Addicus' Legacy Foster and Volunteers page on Facebook ( to find help yourself.



  • All paperwork for the dog should be placed in a Ziploc bag or brown envelope clearly labeled with your dog’s name and "Addicus’ Legacy Dog Rescue". Please also include a note stating dates of last flea/tick and heart worm preventatives.

  • Indicate the type of food you are feeding, how much and when and provide a Ziploc bag with several servings of food for transport. Clearly indicate the dog’s name and "Addicus’ Legacy Dog Rescue" on the bag as well.  

  • Attach tape around the dog’s collar and using a sharpie, write the dog’s name and the following number on it:


  • Morning of transport: limit food - many dogs get scared when they are loaded, and if we can reduce vomit, that would be great :)

  • You may send a blanket or small toy with your foster dog but they have limited space for lots of luggage, so pups and dogs must travel light!!
